 You must submit the required emergency medical form separately once per academic year (fall-spring). Emergency medical forms must be signed by hand before being returned to ILR. Please note:

• Online registration payment is by credit card only.
• All ºÚÁÏÉçÇøs must be registered. Sharing a membership is prohibited.
• You have the option to include additional registrants in your online registration and make a single payment.

Return the emergency medical form to ILR via:
